Augusta, Georgia company, Event Coordinators Inc. has been cited by the North Carolina Labor Department for 12 serious violations after one of their employees was killed in May dismantling a Ferris wheel in North Carolina according to savannahnow.com.
The employee, Michael Douglas Cook, 42, of Columbus, was killed at the end of the six-day carnival.
Safety investigators found a failed fitting on a wire cable used for taking apart the Ferris wheel, which caused part of the Ferris wheel to fall, hitting a 40-foot-high walkway and a basket holding another worker.
